You cant and should not if you dont have help. I am very fortunate as I have had to take time off once a year for the past 3 years to have my hip replaced (3 Xs). Each time I missed less than 2 weeks and my broker stepped up for me while I was out. I have paid her back by bringing her on several co-listings with me. It is all about relationships :) Take some time off, sure you deserve it.

The belief that you can't take a vacation in this business is preposterous. I tie all of the loose ends prior to leaving and give customers a heads up when I'll be back. If you operate under the guise of always being "on call" your efficiency suffers and that can be much more detrimental to your business.

You never know how busy you're going to be at a future date, so you just have to plan the vacation and do your best.
Ask that someone at your office help with things that require someone local. If you've provided assistance to agents in the past, look to those agents to return the favor.
That being said, be prepared to do as much as possible remotely so your reliance on someone else is minimal.
Also, most people are very understanding when their agent is away as long as they're advised and you don't just go silent.
