Usually 1st names are acceptable unless my clients are elderly (+75) then I will always begin with Mr or Mrs and allow them to direct me.

I don't think I have ever in my life been referred to as Mrs and I wouldn't care for it should it ever happen!

It depends on the client.  If they are from another country, there is a protocol to follow especially if they have a title, or if they are from an embassy.  If they were tied to a relighion such as a priest, etc...another protocol needs to be followed.  Most of the time we were happy to use first names, unless there was this type of situations.  A

I am first name but I'll take my lead from the client.  If they introduce themselves as Dr. or Mr.or Mrs, then I'll follow suit until or unless they say call me "John."

Hi Jeff -- it depends on the client.   Typically, first name but there have been a few who have introduced themselves as Dr/Mr/Ms/Mrs/Miss and I follow until informed otherwise.   I'm Michael or Mike -- not typically Mikey -- but I'm likely to try anything....once.

Jeff - Typically I get to know my clients pretty well and just call them by their first name. They also call me by my first name as well.

For me, as I'm Gen X, it is almost always first name.  I think I had one person introduce themselves as a Mrs So and So, but that's it.

I really can't imagine calling someone that way. It just feels distant and wrong.  I have no issue writing Dr blah blah blah in the proposal, but no I would not verbalize it.
