Automated email request for feedback demonstrates an agent who is NOT interested in feedback but one who is simply chasing shiny things.
If I want or another agent actually wants feedback, that request will be shared at the time the agents talk to schedule the showing.
So, many agents don't 'talk' to schedule showings. Again. a don't care agent.

I don't use email feedback forms, but a lot of people do. Good for you for crafting questions that yield more relevant answers.
When the appointment is made, I tell the showing agent I'd really appreciate their feedback and that of their client and that I'll be calling once the showing has concluded or a few hours after.
Most of the calls get answered and I get the feedback. I prefer a phone calls because the conversation can go further. If an agent seems on the fence about asking me something, I can give the agent assurance it's okay to say or ask anything.
