No, but I'm sure to wipe them good (assuming there's a mat upon my entrance) and I do ask if they would prefer they be removed (& if it's nasty out would just do it anyway).  I also make sure to notice what the homeowner is doing and take a cue from them....

Depends on the house. I do not mind removing my shoes and always make  a gesture to do so, unless they insist on keeping my shoes on.
