We call that form, The addendum of death. While we are not lawyers, in my state we are held to the same standard when drafting contracts. Most of what we use are fill in the blanks, but we have a blank form and a few blank paragraphs on other forms where we can take our chances.
Who, what, by when, where, how and what if... are the basics, but I review every one I do with at least one other experienced person before submitting for signatures to ensure what I think it says is what it actually says.

You can add anything you want that may be allowable under contract law in an addendum. Getting the other party to agree is something else. Remember, it takes two to tango in a contract.
