In my 15+ years experience the only reason a property did not close was that the sellers had an FHA loan and did not reach out to me in time to do a short sale before the lender foreclosed. 

One seller changed their mind ...we had 7 offers on the table!

Once I had an easement show up on a survey the title company missed in there initial search. Surprise! It went through the middle of a house on acreage.

Fortunately, It's happened so rarely to me in 25 years that I can't even thing of a situation when it did, but twice when the seller decided at last minute not to accept his corporate relocation and a buyer who was being relocated was fired along with 100 other people one week prior to closing.
