The biggest one has to be getting messages to the other parties to a Contract - example: Today, I received a text message from a seller (I'm repping the buyer). Evidently the other agent wasn't getting the sellers the information they had been requesting so, he (the seller) came straight to me. I got him his answers - to multiple questions. I think once agents get clients under Contract, they forget there is more work to be done. Sad but, true.
The second would have to be inspection items - SO many people (AGENTS) make mountains out of molehills. Agents need to understand, at LEAST, the common items that come up on an inspection report - and what it takes to correct them. Most items are quite minor but, when there are 30 minor items, the list seems to be a little too lengthy for the agents to review with their clients. Combating the inspection items and getting through to the other agent to get his/her party to the finish line is a big challenge!

Helping people take action they would not be able to get themselves to take without outside help. Actions that will benefit them but they are unable to bring themselves to do - through fear of making a mistake or analysis paralysis, or incorrect data. I guess said another way, in my opinion our number 1 problem is getting out clients into a position to succeed.
To me the second is to spare the client the emotional roller coaster ride some transactions bring - have a solution before bringing a problem that has come up to your clients attention.
And third, have plenty of resources for your clients - dependable service oriented service providers - lenders, inspectors, CPA's, attorney's, contractors etc. - this is kind of #2 - have good solutions to situations that come up.

I don't think in terms of problems... only challenges. But at the end of the day, I don't even face all that many challenges... be organized, know what you are doing, control your clients with appropriate expectations set up-front, and the challenges are few and far between.
